Checklist of REQUIRED Documents for Submission
Copy of Post 1969 501(c)(3) (not confirmation letter, but copy of actual 501(c)(3)).
Copy of original letter of determination, copy of any name change(s) and confirmation letter of status if older that 5 years.
Religious Organizations with "blanket" 501(c)(3) IRS letter need to provide copy of page in directory showing your organization.
Most recent 990 for all even if not required to file.
Most recent Audited Financial Statements.
Proposed Budget for project for which funds are requested for year requested.
Organizational Operating Budget for year requested.
Completed Application Form WILL ONLY be accepted by being directly emailed to Patricia Moser at patricia@sterlingturnerfoundation.org . APPLICATION WILL NOT BE ACCEPTED IF ALTERED.

List of Supports and/or potiential supports for the current year and the amount received or requested
