Checklist of REQUIRED Documents for Submission
Copy of Original IRS Determination Letter 501(c)(3) (note, date will be reflected when founded)
Copy of any name change(s) documents from IRS and confirmation letter from IRS (not organization heads)
Religious Organizations with "blanket" 501(c)(3) IRS letter need to provide copy of page in directory showing your organization.
Most recent 990 for all even if not required to file.
Most recent Audited Financial Statements.
Proposed Budget for project for which funds are requested for year requested.(if requesting project/program funds)
Organizational Operating Budget for year requested.
Completed Application Form WILL ONLY be accepted by being directly emailed to Patricia Stilley at patricia@sterlingturnerfoundation.org . APPLICATION WILL NOT BE ACCEPTED IF ALTERED. Please follow instructions posted under application tab.
List of CURRENT Board of Directors. ( including their addresses and phone numbers)
List of Supports and/or potiential supports for the current year and the amount received or requested
509 (a) (3) Letter if required. (A template of this letter can be founded under "Required 509(a)(3) tab. If your organization falls under this category please download, complete and include with your documents.)

ALL documents must be received NO LATER than March 1st by 5:00 p.m. to be considered. Proposals will be accepted between August 1st of the prior year and March 1st by 5 pm.
